Epilepsiechirurgie bei Kindern: Operationsindikationen und Möglichkeiten

Zusammenfassung

Die Akzeptanz der Epilepsiechirurgie als Behandlungsoption bei schweren Epilepsien im Kindesalter ist in den vergangenen Dekaden stetig gestiegen. Spätestens bei nachgewiesener Pharmakoresistenz sollte die Möglichkeit eines epilepsiechirurgischen Eingriffs erwogen und die entsprechende prächirurgische Diagnostik eingeleitet werden. Der epileptologische Erfolg hängt vom Epilepsiesyndrom, den zugrunde liegenden Ursachen und der Abgrenzbarkeit des epileptogenen Areals ab. Mit der Anfallskontrolle sind oftmals entscheidende Verbesserungen der Kognition und des Verhaltens zu beobachten. Insgesamt sollte man bei Kindern mit einer schweren fokalen Epilepsie frühzeitig die Chance überprüfen, durch einen operativen Eingriff bei akzeptablem Risiko Anfallsfreiheit und eine günstigere psychosoziale Entwicklung zu erreichen.
