Zusammenfassung
Der posthämorrhagische Hydrozephalus (PHH) ist eines der großen Probleme der Frühgeburtlichkeit bedingt durch eine intrakranielle/intraventrikuläre Blutung mit residueller Resorptionsstörung des Liquors und/oder Verklebung der Liquorabflusswege mit dem Resultat einer Liquorzirkulationsstörung mit gesteigertem intrakraniellen Druck und damit einhergehender Minderperfusion und Ischämie. Durch das bessere Verständnis der Physiologie der Frühgeborenen und erfolgreicher Behandlung respiratorischer Probleme stieg die Zahl der Überlebenden Frühchen <30. Schwangerschaftswoche und Geburtsgewicht <1500 g in den letzten Jahren stetig an. Die Pathophysiologie ist bis heute jedoch noch nicht vollständig geklärt, sodass auch derzeit keine sicheren prophylaktischen Maßnahmen zur Verhinderung des PHH gibt. Auch fehlen standardisierte Diagnostiken und Therapien. Die Ausprägung des PHH kann vielfältig sein, demzufolge das Outcome auch recht unterschiedlich. Neben motorischen Störungen (wie z. B. die infantile Zerebralparese mit Tetraspastik) können die Kinder auch geistige Beeinträchtigungen oder andere neurologische Störungen entwickeln. Im diesem Kapitel wird die Problematik des PHH inkl. aktueller Therapieoptionen weiter aufgeführt und diskutiert.
