Zusammenfassung
Die primären und sekundären Störungen des kraniozervikalen Übergangs (KZÜ) stellen bezüglich ihrer Ätiologie, der Symptomatik, des klinischen Verlaufs und auch der Behandlungsoptionen eine recht heterogene Gruppe dar. Embryologische Kenntnisse erleichtern das Verständnis dieser Pathologien erheblich. Diagnostisch sind Schnittbildverfahren und funktionelle Bildgebung meistens unverzichtbar. Häufig müssen auch im Kindesalter Computertomographien mit 3D-Rekonstruktionen durchgeführt werden. Die Therapieoptionen umfassen konservative orthotische Verfahren, aber viel häufiger, insbesondere bei Myelonkompression oder Instabilität des KZÜ, operative Methoden. Letztere konnten zuletzt deutlich verfeinert werden, sodass nun auch bei Kleinkindern häufig primär-stabile Versorgungen möglich geworden sind. Die Halofixation ist in Fällen ohne Primärstabilität ein häufig genutztes Zusatzverfahren.
