Zusammenfassung
Die Lungentransplantation ist ein Verfahren, um das Überleben von Patienten mit verschiedenen terminalen Lungenerkrankungen zu verbessern. Sie ist mit 5 Operationen auf 1 Mio. Einwohner in den westlichen Industrieländern eine extrem seltene Therapieform. Sehr seltene Lungenerkrankungen machen 5% aller Indikationen zur Lungentransplantation aus, davon sind die Lymphangioleiomyomatose (LAM), die Bronchiolitis obliterans und die pulmonale Beteiligung bei Kollagenosen die Hauptindikationen. Einige dieser seltenen Lungenerkrankungen haben eine systemische Beteiligung, die zu speziellen postoperativen Problemen nach einer Lungentransplantation führen kann und die vor einer Transplantation sehr sorgfältig geprüft werden muss. Typische postoperative Komplikationen bei LAM umfassen die intraoperative Blutungsneigung, den Chylothorax, den Pneumothorax und die Blutung bei Angiomyolipom. Bei der Langerhans-Zell-Histiozytose (Histiozytosis X, HX) kann es zu extrapulmonalen Manifestationen in den Knochen und in der Hypophyse kommen. Einige seltene Lungenerkrankungen treten typischerweise im Transplantat wieder auf, manche dieser Rezidive sind aber asymptomatisch. Die Langzeitergebnisse großer Transplantationszentren zeigen in den letzten Jahren verbesserte Ergebnisse nach Lungentransplantation. Die Haupttodesursachen nach Lungentransplantation sind das chronische Lungentransplantatversagen und Infektionen. Die Überlebensrate bei Patienten mit seltenen Lungenerkrankungen ist denen von Patienten mit anderen Indikationen nicht unterlegen, teilweise ist sie sogar besser.
