Zusammenfassung
Während früher oft der Begriff Cor pulmonale verwendet wurde, spricht man heute meist von der pulmonalen Hypertonie (PH). Eine PH ist definiert durch einen pulmonalen Mitteldruck ≥25 mmHg in Ruhe, mittels Rechtsherzkatheter gemessen. Für eine relativ kleine Subgruppe der PH-Patienten, solche ohne zugrunde liegende Lungen- oder Herzerkrankung und ohne chronische Thromboembolie oder Sarkoidose, wurde der Begriff „pulmonal arterielle Hypertonie“ (PAH) geprägt. Für die PAH gibt es mehrere zugelassene Therapien, aber für die Non-PAH-PH gibt es keine zugelassene Therapie, wenn man von der nichtoperablen chronisch thromboembolischen PH (CTEPH) einmal absieht. Für die CTEPH ist die Therapie der Wahl die Pulmonalisendarterektomie, die in Expertenzentren gegenwärtig nur noch eine Mortalität >7% aufweist. Die PH bei Lungenkrankheiten wird seit der Weltkonferenz von Nizza, 2013, so eingeteilt, dass bei chronisch obstruktiver Lungenkrankheit (COPD), idiopathischer Lungenfibrose (IPF) und kombinierter pulmonaler Fibrose mit Emphysem (CPFE) von einer schweren PH-COPD/PH-CPFE/PH-CPFE gesprochen wird, wenn in Ruhe ein pulmonaler Mitteldruck ≥35 mmHg vorliegt. Liegt der Druck zwischen 25 und 35 mmHg, lautet die Bezeichnung PH-COPD/PH-IPF/PH-CPFE. Ist bei einer schweren PH-Lungenerkrankung nur eine gering eingeschränkte Lungenfunktion vorhanden, so ist es auch möglich, dass eine leichte Lungenerkrankung und gleichzeitig eine schwere PAH vorliegen. All diese Patienten sollen sich in einem Zentrum vorstellen, welches nicht nur für pulmonale Hypertonie, sondern auch für Lungenkrankheiten ausgewiesen ist. Nach wie vor wird die Diagnose der idiopathischen PAH spät gestellt. Es besteht ein Bedarf sowohl an weiteren nichtinvasiven diagnostischen Methoden als auch an Bewusstsein für diese bedrohliche Krankheit unter den Ärzten und in der Bevölkerung. Luftnot bei Belastung, die nicht durch einen kardialen oder pulmonalen Befund oder durch Hypertonie oder Anämie zu erklären ist, sollte den Verdacht auf eine pulmonale Hypertonie lenken und die Überweisung an ein Expertenzentrum nach sich ziehen.
