Zusammenfassung
Die idiopathische pulmonale Hämosiderose – auch bekannt als Morbus Ceelen – ist ein seltenes alveoläres Hämorrhagiesyndrom ungeklärter Ursache, welches durch die Trias Eisenmangelanämie, Hämoptysen und pulmonale Infiltrate charakterisiert ist. Bereits 1864 beschrieb Virchow das Bild einer „braunen Lungeninfiltration“. Später erfolgte eine weitere Charakterisierung durch Ceelen (1931) und Waldenstrom (1944). Da alveoläre Einblutungen durch zahlreiche Erkrankungen und Noxen ausgelöst werden können, ist die idiopathische pulmonale Hämosiderose (IPH) eine Ausschlussdiagnose. Verlauf und Prognose der IPH variieren erheblich und sind wesentlich abhängig von einer raschen korrekten Diagnose und dem Ansprechen auf eine immunsuppressive Therapie. Ziel dieser Therapie ist die Vermeidung von Rezidiven und der Entwicklung einer Lungenfibrose.
