Blutmanagement in der Orthopädie und Unfallchirurgie

Zusammenfassung

Unter dem Begriff ″patientenorientiertes Blutmanagement″ (PBM) versteht man einen multimodalen, evidenzbasierter Behandlungsansatz. Durch ein individualisiertes 3-Säulen-Konzept sollen Bluttransfusionen vermieden werden, sodass erst nach Optimierung aller anderen Aspekte Blutprodukte leitliniengerecht zur Anwendung kommen. Bei einem Hämoglobinwert unter 12 g/dl bei Frauen bzw. unter 13 g/dl bei Männern liegt eine Anämie vor (WHO-Definition). Konsequentes PBM führt zur Steigerung der Patientensicherheit, da die präoperative Anämie ein deutlich erhöhtes Letalitäts- und Komplikationsrisiko birgt und den größten Prädiktor für die perioperative Verabreichung von Blutkonserven darstellt.
