Abstract

Men idealize their body dominated by a broad-shouldered full upper torso with small flat nipples, a trim rippled abdomen and narrow waist, small rounded buttocks with generalized visible muscularity. The skin with a thin layer of subcutaneous tissue tightly envelops bulging underlying muscles. With the approach and techniques described in this chapter, body contouring surgery in men can achieve that appearance. When lose skin is not an issue, advanced techniques in liposuction can sculpture masculinity. However, when lose skin and excess fat obscure their muscles, body builders are frustrated. As opposed to women who seek sculptured adipose features, men want visibility of their hard-earned muscular contours. Successful male body contouring surgery reduces sagging skin and unwanted fat, eliminates breast fullness, and improves muscular show. The upper body dominates the lower. In a properly selected patient, that result is achieved by a series of interrelated obliquely oriented large elliptical skin excisions across the torso. Current male total body lift surgery consists of a boomerang correction of gynecomastia with a J-torsoplasty, combined with an abdominoplasty with oblique deep excision extensions directly over the flanks.
