Zusammenfassung
Die chronische distale Syndesmoseninsuffizienz ist eine schwerwiegende Pathologie nach Pronations-Eversions- und Pronations-Abduktions-Frakturen. Sie bedingt eine subjektive Instabilität im oberen Sprunggelenk, Schmerzen und Arthrosis deformans. Der subjektive als auch objektive schicksalhafte Verlauf kann durch eine Bandrekonstruktion, die die 3-Punkt-Fixierung der Fibula an die Tibia wiederherstellt, günstig beeinflusst werden. 44 von 45 Patienten gaben nach 5 Jahren eine durchschnittliche Schmerzreduktion von 5 Punkten auf der visuellen Analogskala an, 69 % eine deutliche Verbesserung der subjektiven Stabilität. Objektiv konnten bei allen bis auf einen Patienten die radiologischen Parameter: Medial Clear Space (MCS), Total Fibular Overlapping (TFO) und Total Clear Space (TCS) normalisiert werden. Insofern stellt die von uns inaugurierte Bandrekonstruktion ein sehr gutes Verfahren im Sinne der Meistertechnik dar.
