Zusammenfassung
Das Merkelzellkarzinom ist eine seltene maligne hochaggressive Neoplasie, die bevorzugt an sonnenexponierten Arealen bei älteren Patienten, sowie seltener bei immunsupprimierten jüngeren Personen auftritt. Neben der ultravioletten Strahlung wird daher der Immunsuppression eine zentrale Rolle bei der Pathogenese zugesprochen. In der Mehrzahl der Fäl‐le kann außerdem ein klonal integriertes Polyomavirus (Mer‐kel-Zell-Polyomavirus, MCPyV) nachgewie‐sen werden. Klinisch ist das Merkezellkarzinom ein un‐charakteristischer Tumor. Histopatholo‐gisch zeigen sich monomorphe dermale und/oder subkutane Knoten aus runden bis ova‐len mittelgroßen Zellen mit vesikulärem Nu‐kleus und spärlichem Zytoplasma. Die Zellen des Merkelzellkarzinoms exprimieren Zyto‐keratin (CK) 20 mit punktförmigem („dot-like“) Muster sowie pan-CK, neuroendokrine Marker (u. a. Chromogranin A und Synapto‐physin), Neurofilamentproteine, CD56, CD57, Bcl-2, TdT und PAX-5. Die meisten Fäl‐le zeigen eine Positivität für den gegen MCPyV gerichteten Antikör‐per CM2B4. Tumorgröße und Lymphknotenstatus haben prognostische Bedeutung. Das Vor‐liegen intralymphatischer Tumorkomplexe geht mit einer höheren Rate an Lokalrezidiven und Lymphknotenmetastasen einher. Der Nachweis intratumoraler zytotoxischer T-Lymphozyten in höherer Zahl ist mit einer günstigen Prognose, das Vorliegen von >50 % K-67+-neoplasti‐schen Zellen mit einer schlechten Prognose verge‐sellschaftet.
