Zusammenfassung
Die Psoriasis ist eine chronische, teils immunologisch bedingte papulosquamöse Krankheit mit polygener Prädisposition, kombiniert mit Umweltfaktoren (Traumata, Infekte, Stress, Arzneimittel). Pathophysiologisch spielen Antigen-präsentierende Zellen, T-Lymphozyten, dendritische Zellen und Zellen der angeborenen Immunität zusammen mit Keratinozyten und Mechanismen der Angiogenese eine wesentliche Rolle (Nestlé et al. 2009). Die Psoriasis zählt zu den häufigsten Hauterkrankungen, die Prävalenz liegt zwischen 2–4 %. Eine positive Familienanamnese wurde bei 35–90 % der Patienten gefunden. Die meisten involvierten Gene haben immunologische Kodierungsfunktionen (z. B. für TNF, IFN, IL-23). In bis zu 30 % manifestiert sich die Krankheit an Stellen geschädigter Haut (mechanisch, physikalisch, chemisch, immunologisch), auch isomorpher Reizeffekt oder Koebner Phänomen genannt. Infekte (vor allem Streptokokken) sind insbesondere bei Kindern ein bekannter Auslöser von Erkrankungsschüben. Bei pädiatrischen Patienten beobachtet man häufiger die exanthematischen Formen der Krankheit, bei Erwachsenen vor allem die Plaqueform. Paradoxerweise wird manchmal das Auftreten einer Psoriasis (de novo oder Exazerbation) im Rahmen einer Therapie mit verschiedenen TNF-α-Inhibitoren beobachtet. Auch das Auftreten der palmo-plantaren Pustulose wurde beschrieben. Die Krankheit ist charakterisiert durch eine hyperproliferative Epidermis mit frühzeitiger Reifung der Keratinozyten durch verkürzten Zellzyklus (Mitoserate etwa 8-fach erhöht) mit daraus resultierender Fehlverhornung, die sich durch Präsenz von Kernresten im Stratum corneum (Parakeratose) auszeichnet.
