Zusammenfassung
Bisphosphonate und Denosumab werden bei Osteoporose sowie malignen Erkrankungen des Knochens sehr häufig eingesetzt. Ihre Nebenwirkungen sind deshalb von hoher klinischer Relevanz. Mit einer antiosteoklastischen Behandlung assoziierte Kiefernekrosen manifestieren sich in der Regel als Folge einer zahnärztlichen Behandlung oder akzidentellen Läsion der Mundschleimhaut. Die freiliegenden, avitalen Knochenareale im Kieferbereich mit typischer Symptomatik lassen sich differenzialdiagnostisch gegen andere Erkrankungen der Gesichtsschädelknochen abgrenzen. Die Nekrosen werden überwiegend chirurgisch behandelt – mit oft unbefriedigendem Ergebnis und häufig erheblicher Reduktion der Lebensqualität. Die Folgen einer antiosteoklastischen Therapie am Kiefer können am wirksamsten durch gezielte und standardisierte Prophylaxe reduziert werden. Diese besteht aus individueller Einschätzung von Risikofaktoren, Aufklärung und Instruktion der Patienten, prätherapeutischer Zahnsanierung sowie engmaschigen Kontrollen bei gesicherter interdiszi plinärer Kooperation.
