Zusammenfassung
Über Jahrzehnte bewährte vaginale und abdominale Operationsstrategien mit Eigengewebe zeigen zwar akzeptable Ergebnisse mit moderaten Nebenwirkungen, jedoch auch bis 30% Rezidive aufgrund der bestehenden Bindegewebsinsuffizienz. In den vergangenen Jahren etablierte Methoden mit Verwendung von alloplastischen und biologischen Netzen haben vor allem die anatomische Erfolgsrate signifikant erhöht. Doch neben nur moderaten subjektiven Verbesserungen können neue, zum Teil gravierende Nebenwirkungen, wie Netzarrosionen bis zu 20%, Schmerzsyndrome, Dyspareunie, Infektionen, Obstruktionen und Harndrangsyndrome. Aktuelle Leitlinien bestätigen den Einsatz von Netzen in der Rezidivsitua tion und in individuellen ausgeprägten Primärfällen; für den generellen Einsatz in der Primärsituation des Prolapses gibt es jedoch keinerlei Evidenz. Eine bessere Ausbildung der Operateure, subtile Diagnostik und individuelle umfassende Aufklärung über die geplante Operation und mögliche Alternativen sowie die Überprüfung der Materialien in randomisierten Studien vor Markteinführung helfen zu einer Verbesserung der postoperativen Ergebnisse und Absenkung von Komplikationen und Nebenwirkungen beizutragen.
