Zusammenfassung
Die organerhaltende Hodentumorenukleation stellt die leitliniengerechte Therapie der Wahl bei synchronen oder metachronen bilateralen testikulären Keimzelltumoren oder benignen Hodentumoren dar. Voraussetzung für die Organerhaltung sind eine ausreichende Hodenparenchymreserve des verbleibenden Resthodens nach Enukleation sowie normwertige Serumkonzentrationen für Testosteron und luteinisierendes Hormon (LH). Aufgrund der im Restparenchym immer nachweisbaren intraepithelialen testikulären Neoplasie sollte eine adjuvante Radiation mit 20 Gy angeschlossen werden. Werden diese Kautelen berücksichtigt, bleibt eine physiologische endokrine Hodenfunktion bei über 80 % der Patienten nach einer Nachbeobachtung von 75 Monaten erhalten. Das lokale Rezidivrisiko ist nach adjuvanter Radiation praktisch nicht existent. Die Organerhaltung bedingt kein erhöhtes Metastasierungsrisiko im weiteren Krankheitsverlauf.
