Zusammenfassung
Die Einteilung der Anämien erfolgt anhand klinischer Gesichtspunkte in akut und chronisch, anhand der Erythrozytenindizes in mikrozytär, normozytär und makrozytär, anhand der Retikulozytenzahlen in hypo- und hyperregeneratorisch. Die Diamond-Blackfan Anämie (DBA) ist die einzige angeborene Form der hypoplastischen Anämien, (engl. pure red cell aplasia, PRCA), sie ist von der häufigeren transitorischen Anämie des Kindesalters (TEC) ab zu grenzen. Während Patienten mit DBA und TEC sich durch einen langsamen Abfall der Hämoglobinkonzentration auch an ausgeprägte Anämien gut adaptieren können, kann die aplastische Krise bei Parvovirus-B19-Infektion für Patienten mit hämolytischen Anämien eine Notfallsituation darstellen. Dyserythropoetische Anämien sind seltene angeborene Erkrankungen, die durch eine ineffektive Hämatopoese zur Hämosiderose führen können. Mikrozytäre Anämien werden durch eine unzureichende Füllung der Erythrozyten mit Hämoglobin, also letztlich durch eine Hämoglobinsynthesestörung, verursacht. Die häufigsten mikrozytären Anämien sind die Eisenmangelanämie und die Thalassaemia minor. Den makrozytären Anämien liegt in der Regel eine tiefgreifende, exogen oder endogen bedingte Störung der Erythropoese und oft auch der gesamtem Hämatopoese zugrunde. Hämolytische Anämien zeichnen sich durch einen erhöhten Erythrozytenabbau aus. Dabei werden Bilirubin und Laktatdehydrogenase als »Hämolyseparameter« freigesetzt, Haptoglobin wird durch das vermehrt anfallende freie Hämoglobin verbraucht. Im letzten Abschnitt schließlich werden die Hämoglobinopathien besprochen. Hier liegen Mutationen der Globingene zugrunde.
