Zusammenfassung
Harninkontinenz ist eine häufige Komorbidität bei Patienten mit COPD. Chronischer Husten steigert den Druck im Bauchraum, und bewirkt dadurch einen erhöhten Druck auf die Blase, der den Schließmechanismus der Blase beeinträchtigt. Das physiotherapeutische Assessment erfasst die Problematik mit krankheitsspezifischen Fragebögen, standardisierten Pad-Tests und einem Miktionstagebuch. Muskelfunktionstests dienen der Beurteilung der Beckenbodenmuskulatur und der Erfassung der Inkontinenzsymptome. Das physiotherapeutisch geleitete Kontinenztraining umfasst die Patientenedukation mit dem Ziel der Optimierung des Trink- und Entleerungsverhaltens. Das Training der Beckenbodenmuskulatur dient der Schulung der Sensomotorik, sowie der Verbesserung der Ausdauerleistung, Reaktionsfähigkeit und Kraft. Sinnvoll ist eine Integration der Beckenbodenkontrolle in Alltagsbewegungen, insbesondere bei Husten. Um eine langfristige Verbesserung der Blasenkontrolle zu erzielen sollte anschließend ein Erhaltungstraining als Bestandteil der Lungenrehabilitationsprogramme möglichst in der Gruppe durchgeführt werden.
