Zusammenfassung
In den meisten Fällen degenerativer HWS-Erkrankungen liegt eine Kompression nervaler Strukturen von ventral vor, somit wird am häufigsten der anteriore Zugang angewandt. Der posteriore Zugang eignet sich für Situationen mit ligamentärer Hypertrophie und raumfordernder Facettengelenksarthrose sowie für ältere Patienten mit knöcherner Spontanfusion im Bereich der Zwischenwirbelräume, wobei eine Lordosierung der HWS eine notwendige Voraussetzung für den Zugang von dorsal ist. Kombinierte Eingriffe sind indiziert bei Korpektomien von mehr als zwei Wirbelkörpern sowie beim Vorliegen metabolischer Erkrankungen, die mit einer verminderten Knochenqualität einhergehen (z. B. Osteoporose, Diabetes mellitus, Niereninsuffizienz), hierbei ggf. auch Korpektomien in ein oder zwei Höhen.
