Palliative Maßnahmen beim hilusnahen inoperablen malignen Gallengangsverschluß unter besonderer Berücksichtigung der direkten intrahepatischen Cholangiojejunostomie nach Longmire

Summary

The modification of the method reported by Longmire and Sanford for the internal bile drainage, which was reported by the author in 1967, is suitable as a palliative drainage method for high, inoperable primary or metastatic occhisions of the common hepatic duct by a tumor. A wedge resection from the right or left lobe of the liver is necessary for this. A bile duct which is found deep inside the liver and which has at least a diameter of 6 to 8 mm is directly anastomosed with a loop of proximal jejunum which has been shunted according to the method of Roux. In 4 Patients construction of this anastomosis was quite successful and the obstructive jaundice was thus eliminated. In 2 other patients the diameter of the biliary tracts which were found intrahepatically was too small. In a 7th patient with infiltrating adenocareinoma of the bifurcation of the hepatic duct drainage of bilde could only be achieved by intraluminal insertion of a plastic tube into the left lobe of the liver.

Zusammenfassung

Die vorn Verfasser 1967 angegebene Modifikation der von Longmire und Sanford beschriebenen Methode zur inneren Gallendrainage eignet sich als palliatives Drainageverfahren beim hohen inoperablen, primären oder metastatischen Tumorverschluß des D. hepaticus communis. Es ist dazu eine Keilexcision aus dem rechten oder linken Leberlappen notwendig. Ein in der Tiefe aufgespürter Gallengang von mindestens 6–8 mm Durchmesser wird mit einer nach Roux ausgeschalteten oberen Jejunumschlinge direkt anastomosiert. Bei 4 Patienten gelang die Herstellung dieser Verbindung sehr schön, so daß der Verschlußikterus behoben wurde. Bei 2 weiteren Kranken war das Kaliber der intrahepatisch aufgefundenen Gallengänge zu gering. Bei einem 7. Kranken mit infiltrierendem Adenocarcinom der Hepaticusbifurkation konnte die Ableitung der Galle nur aus dem linken Leberlappen durch intraluminales Einlegen eines Plastiktubus erreicht werden.
