Summary
During a period of 10 years 66 secondary procedures were performed on the extrahepatic biliary tract. 42 times stones in the common bile duct were made responsible for this, 11 times a rigid papilla, 6 times residual conditions after T-tube drainage and 7 times other causes were held responsible for this. The number of secondary procedures can be definitely decreased by paying attention to the following postulates:

1.
Early treatment of patients with gall stones.

2.
The use of radiomanometry during every cholecystectomy, and
-
3.
Primary closure of the common bile duct after revision of the biliary tract, i. e. foregoing routine drainage with a T-tube.
Zusammenfassung
In 10 Jahren wurden 66 Zweiteingriffe an den extrahepatischen Gallenwegen ausgeführt. 42 mal wurden Steine im Choledochus, 11 mal eine rigide Papille, 6 mal Folgezustände nach einer T-Drainage und 7 mal andere Ursachen dafür verantwortlich gemacht. Die Zahl der Zweiteingriffe läßt sich deutlich senken, wenn folgende Forderungen beachtet werden:

1.
Die Frühsanierung des Gallensteinkranken.

2.
die Anwendung der Radiomanometrie bei jeder Cholecystektomie und
-
3.
der primäre Choledochusverschluß nach Gallengangsrevision, d.h. der Verzicht auf die routinemäßige T-Drainage.
