Summary

Excision of a gall bladder without stones has always been a controversial surgical procedure. Of 1500 consecutive cholecystectomies 320 were primary hyperplastic cholecystoses and this accounts for 21.3%. The commoner forms of these were hyperplasia of the mucosa, cholesterolosis and adenomyomatosis.

A common aspect of all of these types is the typical gall bladder history and the typical picture of biliary disease and in this connection mainly the pain and the food intolerance decide the method of treatment. The diagnosis can usually only be made during surgery or under the microscope. On account of this the importance of exact history taking and the absolute necessity of an examination of the neighbour organs must be emphasized. 77% of these 320 cases became asymptomatic and thus surgical treatment is justified.

Die Entfernung einer steinlosen Gallenblase ist immer ein umstrittener Eingriff gewesen. — Unter 1500 konsekutiven Cholecystektomien befanden sich 320 primäre hyperplastische Cholecystosen, was 21,3% ausmacht. Von diesen waren die gewöhnlichsten Formen Hyperplasie der Schleimhaut, Cholesterolose und Adenomyomatose.

Allen diesen Formen ist eine typische Gallenanamnese und das klinische Bild der Gallenerkrankungen gemeinsam, in denen vor allem die Schmerzen und die Nahrungsintoleranz den Weg der Behandlung entscheiden. Die Diagnose kann gewöhnlicherweise erst während der Operation oder im Mikroskop festgestellt werden. Aus diesem Grund ist die Bedeutung einer genauen Anamnese und die absolute Notwendigkeit einer Untersuchung der Nachbarorgane zu unterstreichen. Von den 320 Fällen wurden 77% symptomlos und damit berechtigt zur operativen Behandlung.
