Die operative Behandlung des Ischias-Syndroms

Summary

This presentation deals almost exclusively with the surgical treatment of prolapse of the lumbar intervertebral disks. As an introduction the author presents the indications for a surgical approach and subsequent to this he describer the various possibilities of the surgical methods. At the present time removal of the ligamentum flavum with additional, minimal excision of adjacent parts of the arch have generally been found to be an adequate approach. Hemilaminectomy or laminectomy is only necessary in exceptional cases. The small vertebral joints are not touched. In addition to removal of the visible prolapse of the intervertebral disk this is also additionally debrided as much as possible. Artificial replacement of the removed intervertebral disk tissue by plastic or metal spheres does not offer additional advantages. In exceptional cases instability of the corresponding motor segment of such severity that fusion operation becomes necessary may occur after surgical procedures on the intervertebral disk.

Zusammenfassung

Das Referat beschäftigt sich praktisch ausschließlich mit der operativen Behandlung des lumbalen Bandscheibenvorfalles. Zunächst werden die Indikationen für ein operatives Vorgehen dargestellt und anschließend die verschiedenen Möglichkeiten des operativen Geschehens selbst geschildert. Heute hat sich allgemein eine Entfernung des gelben Bandes mit zusätzlicher geringer Eortnahme angrenzender Bogenanteile als ausreichender Zugangsweg erwiesen. Nur im Ausnahmefall ist eine Hemilaminektomie oder eine Laminektomie notwendig. Die kleinen Wirbelgelenke bleiben unbehelligt. Außer der Entfernung des sichtbaren Vorfalles der Bandscheibe wird diese auch zusätzlich so weit wie möglich ausgeräumt. Der künstliche Ersatz des entfernten Zwischenwirbelscheibengewebes durch Kunststoff oder Metallkugeln bietet keine zusätzlichen Vorteile. Im Ausnahmefall kommt es nach einer Bandscheibenoperation zu einer solchen Instabilität im entsprechenden Bewegungssegment, daß eine Versteifungsoperation notwendig wird.
