Die Stellung des Trasylols in der Behandlung der akuten Pankreatitis

Summary

The status of Trasylol in the treatment of acute pancreatitis is discussed. The results of the writer’s experiments indicate that Trasylol in acute haemorrhagic reflux-pancreatitis was able to prevent lethal shock and to stop the continuous fall of blood pressure. The mortality in the experimental model was lowered from 100% to 55% by administration of Trasylol. The effect of Trasylol is thought to be due essentially to kininogen-kinin activation. The writer’s own investigations have shown that immediately before the onset of acute pancreatitis the peripancreatitic lymph vessels are full of cell detritus that flows in a central direction. This cell detritus contains shock-producing kinins, which are successfully inactivated by Trasylol. Trasylol is an integral component of the modern treatment of pancreatitis.

Zusammenfassung

Die Stellung des Trasylols in der Behandlung der akuten Pankreatitis wird diskutiert. Anhand eigener experimenteller Ergebnisse konnte nachgewiesen werden, daß das Trasylol bei der akuten hämorrhagischen Reflux Pankreatitis den tödlichen Schock abfangen und den kontinuierlichen Blutdruckabfall aufheben kann. Durch die Gabe von Trasylol kann bei diesem experimentellen Modell die Mortalität von 100 auf 55% gesenkt werden. Die Wirkung des Trasylols wird im wesentlichen mit einer Kininogen-Kinininaktivierung gesehen. Wie durch eigene Untersuchungen nachgewiesen werden konnte, sind die peripankreatitischen Lymphgefäße unmittelbar nach Auslösen der akuten Pankreatitis mit Zelldetritus vollgefüllt, der in kräftigem Strom nach zentral zu abtransportiert wird. In diesem Zelldetritus sind die schockauslösenden Kinine enthalten, die durch das Trasylol erfolgreich inaktiviert werden. Das Trasylol wird als integrierter Bestandteil der modernen Behandlung der Pankreatitis angesehen.
