Die Herztransplantation als Behandlungsmöglichkeit der schweren Coronarinsuffizienz

Summary

Cardiac transplantation provides a new method for the treatment of advanced cardiac disease. Since May 1968 17 cardiac transplantations in 16 patients have been performed, using allografts, at the Texas Heart Institute. In 12 cases the operation was indicated on account of heart failure due to diffuse coronary sclerosis, resistant to therapy. At the present time, 2 of these patients, 5 and 9 months after the operation, are still alive. Rejection and infection were the main causes of the 3 early and 7 late fatalities. During the same period of time 35 patients with severe heart failure could not receive a transplant since a donor was not available. 50% of these did not survive a waiting time of 1 month and only 14% lived longer than 6 months. A comparison of the patients who had received transplants and the potential recipients who did not undergo surgery shows that hitherto the patients who had received transplants had, on the average, survived for a longer period of time. Nevertheless, at the present time world-wide clinical use of this method appears to be premature. However, in special cases cardiac transplantation is justified as a palliative treatment possibility for severe coronary insufficiency.

Zusammenfassung

Die Herztransplantation eröffnet einen neuen Weg in der Behandlung fortgeschrittener Herzkrankheiten. Seit Mai 1968 wurden am Texas Heart Institut unter Verwendung von Allografts 17 Herztransplantationen bei 16 Patienten durchgeführt. In 12 Fällen bildete eine therapieresistente Myokardinsuffizienz auf der Basis einer diffusen Coronarsklerose die Indikation. Zur Zeit leben noch 2 dieser 12 Patienten 5 und 9 Monate nach der Operation. Abstoßung und Infekte bildeten die Hauptursachen der 3 Früh-und 7 Spättodesfälle. In der gleichen Zeitspanne konnten 35 Patienten mit schwerer Coronarinsuffizienz zufolge Spendermangels nicht transplantiert werden. Von diesen haben 50% eine 1 monatige Wartezeit nicht überlebt, und nur 14% lebten mehr als fi Monate. Eine Gegenüberstellung der transplantierten Patienten und der nicht operierten potentiellen Empfänger zeigt, daß bisher die transplantierten Patienten im Durchschnitt länger gelebt haben. Trotzdem erscheint eine weltweite klinische Anwendung der Methode zum jetzigen Zeitpunkt noch verfrüht. ln speziellen Fällen ist die Herztransplantation als palliative Behandlungsmöglichkeit der schweren Coronarinsuffizienz jedoch gerechtfertigt.
