Zusammenfassung
Der Wert einer broncholytischen Therapie bei obstruktiven Ventilationsstörungen wird letztlich an der Besserung der arteriellen Blutgase gemessen. Sinnvoll erscheint eine solche Behandlung nur, wenn die Verminderung der Atemarbeit über eine verbesserte alveoläre Ventilation auch die arterielle Hypoxie und Hyperkapnie bessert. Veröffentlichungen der letzten Jahre haben gezeigt, daß Aminophyllin, Atropin und auch Adrenalin trotz guter und lang anhaltender bronchospasmolytischer Wirkung [7] den arteriellen Sauerstoffdruck nicht erhöhen, häufig sogar erniedrigen [1, 3, 8, 11, 12]. Diese scheinbar paradoxe Reaktion des arteriellen 02-Druckes wurde auf eine Störung des Ventilations-Perfusionsverhältnisses und eine vermehrte pulmonale Kurzschlußdurchblutung zurückgeführt, ohne daß dieses direkt bewiesen werden konnte.
