Abstract

The reported occurence of so called ‘false positives’ in a wide range of neurological diseases threatens to discredit the reputation of the oligoclonal reaction as a diagnostic test for multiple sclerosis. Selection of patients and the kind of method chosen for separation and detection of oligoclonal bands (OB) will greatly influence the results. Criteria for oligoclonal pattern evaluation have not been properly defined and currently one, two or three OB are taken for a positive rating. Because of the higher resolution the proved method of isoelectric focusing on 1 mm thick Polyacrylamide (pH 3.5–9.5) gels with silverstaining is still preferred to the often recommended separation on agarose with subsequent immunoblotting. The alleged advantage of specific detection of IgG could not be confirmed for electroblotted Polyacrylamide gels.
