Die diabetische Osteoarthropathie

Zusammenfassung

Unter den Folgeerkrankungen des Diabetes sind Knochenaffektionen relativ selten und deswegen auch wenig bekannt. Zweifellos sind sie jedoch häufiger als sie diagnostiziert werden. Nicht selten gehen sie unter anderen Diagnosen wie Arthrosen, Osteomyelitis usw. unter. Die einschlägige Literatur ist, gemessen an der immensen Zahl von Publikationen über den Diabetes, relativ spärlich. Die diabetischen, neuropathischen Osteoarthropathien umfassen auch Spontanfrakturen und Osteolysen ganzer Knochen. Osteolysen und Spontanfrakturen können auch solitär auftreten, ohne daß gleichzeitig an den benachbarten Gelenken destruktive und deformierende Veränderungen bestehen. Ebenso können aber Arthropathien auch ohne Spontanfrakturen und ohne Osteolysen einhergehen. Die diabetische, neuropathische Osteoarthropathie kann man sowohl als Überbegriff für sämtliche drei Detailmanifestationen diabetischer Knochenveränderungen gebrauchen als auch unter ihnen ausschließlich Gelenkveränderungen verstehen; im letzteren Fall spricht man am besten von neuropathischen diabetischen Arthropathien im engeren Sinn.
