Summary

The large intestine is a less frequent source of hemorrhage than is the upper gastrointestinal tract. In a brief survey the surgical problems of colonic hemorrhage are analyzed: severe hemorrhage is usually due to diverticulosis, and much more rarely to colonic cancer. Mild or moderate bleeding is caused, in descending order of frequency, by colonic cracinoma, polyps, ulcerative colitis, Crohn’s disease, and diverticulosis. Mild hemorrhage requires accurate diagnosis. With severe bleeding, conservative therapy should be tried first; if surgical intervention becomes necessary exact localisation of the bleeding source may be very difficult.

Zusammenfassung

Blutungen aus dem Dickdarm sind insgesamt seltener als Blutungen aus dem oberen Gastrointestinaltrakt, häufiger aber Zeichen eines malignen Geschehens. Starke Dickdarmblutungen werden hervorgerufen durch Divertikulose, wesentlich seltener durch M. Crohn, Colitis ulcerosa und nur selten durch ein Carcinom. Die Ursachen für leichte Blutungen sind etwa der Häufigkeit nach Carcinom, Polypen, Colitis ulcerosa, M. Crohn, Divertikulose. Schwache Blutungen erfordern eine einwandfreie Diagnose, starke sind zunächst meist konservativ, in seltenen Fällen operativ zu behandeln, wobei die Lokalisations-diagnostik der Blutung schwierig sein kann.
