Summary

Only the surgical elimination of the source of the trouble brings about conditions in which conservative intensive care can be successful in acute pancreatitis. Subtotal resection (Child) and intensive drainage of all the necrotic areas should be performed as early as possible. This treatment saved 6 out of 10 patients suffering from haemorrhagic pancreatitis. Correct selection of patients and of the right time for surgery and the pre- and postoperative therapy requires close cooperation of surgeons and physicians.

Zusammenfassung

Erst die Ausrottung des Katastrophenherdes schafft bei der akuten Pankreatitis die Voraussetzungen für eine erfolgreiche konservative Intensivtherapie. Möglichst frühzeitig sollten eine subtotale Linksresektion und ausgiebige Drainage aller Nekrose-straßen durchgeführt werden. Mit diesem Vorgehen konnten 6 von 10 Patienten mit einer hä-morrhagisch-nekrotisierenden Pankreatitis gerettet werden. Indikationsstellung, Wahl des Operations-Zeitpunktes, prä- und postoperative Therapie erfordern eine enge Zusammenarbeit zwischen Chirurgen und Internisten.
