Pathophysiologische Reaktionen des kindlichen Organismus bei Mehrfachverletzungen

Summary

Multiple injuries in childhood are observed particularly frequently in the age group 4–6 years. Our own material included 177 children with multiple injuries observed over the period 1964–1973. The reaction of the organism to multiple injuries is very much more pronounced in childhood than in adulthood; the small blood volume leads to underestimation of the extent of the blood loss. High pulse rates and low blood pressure together with the low urea volumes that are normal in childhood can lead to excessive volume replacement while the relative instability of the water balance, the rapid fall in temperature due to the large body surface area, the low level of free fluid reserves available for autoregulation and the low reserves in the buffer system together with the organism’s pronounced susceptibility to acid substances demand checks on the values important to diagnosis at short intervals and correspondingly rapid institution of necessary therapeutic measures.

Zusammenfassung

Von Mehrfachverletzungen im Kindesalter wird insbesondere das 5.–7. Lebensjahr betroffen. Von 1964–1973 wurden im eigenen Krankengut 177 mehrfachverletzte Kinder beobachtet. Die Reaktion des kindlichen Organismus auf eine Mehrfachverletzung ist sehr viel empfindlicher als die des Erwachsenen. Das geringe Blutvolumen führt zu einer Unterschätzung des Blutverlustes. Die hohen Pulswerte und niedrigen Blutdruckwerte sowie geringen Harnsollvolumina können zu übermäßigem Volumenersatz verführen, die relative Wasserinstabilität, der rasche Temperaturverlust bei großer Körperoberfläche, die geringen freien Flüssigkeitsreserven zur Autoregulation und die geringen Reserven im Puffersystem mit schnellem Anfall an sauren Substanzen verlangen kurzfristige Kontrollen diagnostischen Werte und dementsprechend ein rasches Einsetzen therapeutischer Maßnahmen.
