Hiatusbruch, Sphincterinsuffizienz, Refluxoesophagitis — Einleitung und Fragestellung

Summary

The pathologic nature of hiatus hernia is discussed. The indications for surgery should depend only on the severity of reflux esophagitis; this is indicated by the tonus and reactivity of the lower esophageal sphincter and the motility of the esophagus. The pathogenesis of sphincter insufficiency is also discussed, and it is felt that disturbances in hormonal regulation and in nervous factors might be of importance. Manometry has yielded new evidence, and the author discusses what statements this technique allows on diagnosis and on the assessment of the result of surgery.

Es wird die Frage nach dem Krankheitswert der Hiatushernie auf-geworfen. Indikatorische Konsequenzen sollten sich nur auf Grund des Schweregrades einer Refluxoesophagitis ergeben. Hierfür sind Tonus und Reaktivität des unteren Oesophagus-sphincters sowie die Motilität der Speiseröhre entscheidend. Frage nach der Pathogenese der Sphincterinsuffizienz, wobei Störungen einer hormonalen Regulation und nervaler Faktoren bedeutungsvoll sein dürften. Neue Erkenntnisse hat die Manometrie erbracht. Welche Aussagen ermöglicht sie für die Diagnostik und die Beurteilung des Operationserfolges.
