The designation, “Beginning Dust (Pneumoconiotic) Lung”, comprises the early stages following massive dust exposure as well as the dust lung I and grades I and II following lengthy but insignificant exposure.

Dust lungs, especially the mixed dust pneumoconioses I and grades I–II, triggered by obstructive emphysema, lead to a loss of the respiratory reserves with concomitant increased susceptibility to infection and to overtaxing of the right ventricle. The presenting symptom is dyspnea upon effort.

There is no apparent illness with beginning pneumoconiosis. Nevertheless, each additional injury to the parenchyma and circulatory disturbance — especially emboli — does lead to cardio-respiratory insufficiency.

Further, beginning pneumoconiosis I and grades I–II enhance the development of a post-primary complex bacillary tuberculosis; progressive development of a late primary tubercular pulmonary infection is especially favored.

Reflecting the factual increase of pulmonary cancer, the number of cases of dust lungs associated with lung cancer have also increased. Silico-tuberculosis has been replaced by silico-carcinoma. The question of the carcinogenic activity of quartz-containing dust should be re-examined. The carcinogenic properties of asbestos particles have been demonstrated, the average latency period amounting to 20 years.

Die Bezeichnung „beginnende Staublunge“ umfaßt sowohl die Frühstadien nach massiver Staubexposition, als auch die Staublungen I. und I.–II. Grades nach langfristiger, aber nur geringer Staubexposition.

Staublungen, insbesondere Mischstaub-Pneumokoniosen I. und I.–II. Grades führen über das obstruktive Emphysem zu einem Verlust der Atmungsreserven mit einer erhöhten Infektanfälligkeit und einer Mehrbelastung der rechten Herzkammer. Führendes Symptom ist die Atemnot bei Anstrengung.

Ein Krankheitswert kommt der unkomplizierten beginnenden Pneumokoniose nicht zu, doch führt jede zusätzliche Parenchymschädigung und Zirkulationsstörung, im besonderen die Lungenembolie, zur kardiorespiratorischen Insuffizienz.

Auch beginnende Pneumokoniosen I. und I.–II. Grades begünstigen die Entwicklung einer postprimären Zusatztuberkulose und fördern im besonderen die progrediente Entwicklung der pulmonalen tuberkulösen Späterstinfektion.

Im Rahmen der säkularen Zunahme der Lungencarcinome haben auch die Kombinationsfälle Staublunge/Lungencarcinom zugenommen. Die Silikotuberkulose ist heute durch das Silikocarcinom abgelöst worden. Die Frage der carcinogenen Wirkung quarzhaltiger Staube ist erneut zu überprüfen. Für Asbeststaub ist die carcinogene Wirkung erwiesen. Die mittlere Latenzzeit beträgt 20 Jahre.
