Les myomes utérins: Modalités et indications du traitement coelioscopique

Résumé

Seuls les fibromes compliqués et/ou résistants aux traitements médicaux relèvent de la chirurgie. Deux types d’interventions peuvent être proposées pour les myomes interstitiels et sous séreux: la myomectomie et l’hystérectomie. L’indication, posée après un bilan pré-opératoire soigneux, dépend de l’âge de la patiente, de la taille, du nombre et de la localisation des myomes ainsi que des lésions associées. Les progrès réalisés ces dernières années en matière de coeliochir-urgie permettent, à partir de séries préliminaires, de pouvoir affirmer la faisabilité de ces deux interventions par voie coelioscopique. Les années à venir permettront, à partir de séries plus importantes de patientes, de préciser la place respective de chacune de ces interventions par rapport aux autres possibilités chirurgicales.

Abstract

Only complicated fibromas refractory to medical treatment should be treated surgically. Two types of operations can be proposed for interstitial and sub-serosal fibromas: myomectomy and hysterectomy. The indication, based on a through preoperative assessment, depends on the patient’s age and the size, number and sites of the fibromas and associated lesions. Preliminary series confirm the feasibility of these two operations performed by laparoscopy, as a result of the progress in this modality over recent years. In the future, larger series will determine the respective place of each of these surgical procedures in relation to others surgical possibilities.
