Zusammenfassung
Die A. hepatica hat ihre eigene Gefäßchirurgie! Sie ragt aus dem Gefüge der übrigen Abdominalarterien kraft ihrer biologischen und funktioneilen Besonderheiten heraus und bietet auch in anatomischer Hinsicht viel Bemerkenswertes. Die A. hepatica ist ja nicht nur das nutritive Gefäß der Leber, sondern obendrein für die Pfortader und ihr Vorflutgebiet ein wichtiges Druckregulationsorgan. Auf der einen Seite beeinflußt zwar der Pfortaderkreislauf, eingeschaltet zwischen den Blutstrom des Darmes und der Leber, die hämodynamischen Verhältnisse des Bauchraumes maßgeblich, auf der anderen Seite steht aber die V. portae, wie man aus den Untersuchungen von Schwiegk (1932) weiß, mit der Leberarterie in kompensatorischen Wechselbeziehungen, die sich nicht nur auf die Druckverhältnisse, sondern auch auf die Sauerstoffzufuhr zur Leber erstrecken. Nach Ligatur der A. hepatica steigt schon unter Normalbedingungen der Druck im Pfortadergebiet um 10–20mm Wasser an. Ist aber die Leber cirrhotisch verändert, dann genügen geringe Erhöhungen des arteriellen Druckes, um den portalen Überdruck noch mehr zu steigern. Auf diesen Erkenntnissen basiert die chirurgische Konzeption der Arterienligatur beim Pfortaderhochdruck. Sie bezweckt letzten Endes nichts anderes, als den stabilisierenden Effekt der Arterie auf den Pfortaderhochdruck auszuschalten (Abb. 142).
