Abstract
The root of failure in a system as complex and multifaceted as a state-run purchase of service (POS) system is often difficult to determine. When things run well, government administration is quick to claim credit. But when the agencies providing needed human services are plagued by organizational failure and bankruptcy, a mad scramble ensues to determine where the blame should lie.
