Zusammenfassung
Dass der Venenbypass bei der Kniegelenksüberbrückung unangefochten das Bypass-verfahren der 1. Wahl ist, ist eines der therapeutischen Manifeste in der Gefäßchirurgie wohl weltweit. Demgegenüber besteht der weit verbreitete Glaube, dass bei der Bypassanlage supragenuidal, also beim PI-Bypass, das Bypassverfahren der Wahl der Kunststoffbypass ist. Diese Denkweise, die Vene aufzuheben für einen evtl. später erforderlichen infragenuidalen Bypass oder für einen aortokoronaren Bypass, sitzt so fest in den meisten Köpfen der Gefäßchirurgen, dass diese Behauptung überhaupt nicht angezweifelt wird. Das Erstaunliche dabei ist, dass es eigentlich nur 2 Studien von Rosen et al. [10] und Yashar et al. [14] gibt, die diese Behauptung bedingen. Diese beiden Studien konnten keinen signifikanten Vorteil für Vene beim PI-Bypass zeigen, weshalb daraus abgeleitet wurde, bei supragenuidalen Bypässen primär einen Kunststoffbypass zu verwenden. Hält diese Behauptung heute überhaupt noch stand? Warum hat sich diese Behauptung so etabliert? Ist es vielleicht bequemer in den Schrank zu greifen und schnell eine Kunststoffprothese einzusetzen? Wo liegen das Pro und Kontra für Vene vs. Kunststoff beim PI-Bypass?
