Zusammenfassung
Mehrere große multizentrische randomisierte Studien haben die Effizienz der Carotisendarteriektomie (CEA) bei der symptomatischen extrakraniellen Carotisstenose belegt [2, 8]. Obwohl auch bei der asymptomatischen Carotisstenose positive Studiendaten vorliegen [3], ist der Nutzen eher marginal und wird v.a. von den meisten Neurologen in Zweifel gezogen [4]. Ein wesentlicher Faktor bezüglich des Nutzens der gefäßchirurgischen Therapie ist die strenge Abhängigkeit von der perioperativen Mortalität und Morbidität. Sie sollte für die symptomatische Carotisstenose unter 6% und die asymptomatische Carotisstenose unter 3% liegen. Dabei muss berücksichtigt werden, dass in diesen Studien ein selektiertes Patientengut untersucht wurde, insbesondere wurden Patienten mit hoher koronarer Komorbidität (instabile Angina pectoris, nicht länger als 6 Monate zurückliegender Myokardinfarkt, manifeste Herzinsuffizienz, unkontrollierte Hypertonie und Diabetes) ausgeschlossen.
