Zusammenfassung
Die „Response-to-injury-Hypothese“ von Russel Ross [4] fasst eine Vielzahl zellulärer und molekularer Prozesse der Pathogenese der Arteriosklerose zusammen. Am Anfang steht eine Läsion des Endothels, was eine Aktivierung der Gerinnungskaskade und eine Einwanderung von Leukozyten aus dem Blutstrom in die Gefäßwand zur Folge hat. Am Ende dieses Prozesses steht eine komplexe Läsion, die aufgrund von Komplikationen Anlass zur klinischen Symptomatik, d.h. zum akuten Gefäßverschluss geben kann. Hierbei stehen die Ausdünnung der fibrösen Kappe mit nachfolgender Plaqueruptur sowie die Einblutung aus Mikrogefäßen innerhalb der Plaque im Vordergrund. Zahlreiche molekulare Mediatoren, die zur Progression der Plaque beitragen, wurden in den vergangenen beiden Jahrzehnten identifiziert Einer der zentralen Faktoren des Gefäßumbaus ist der Wachstumsfaktor PDGF („platelet-derived growth factor“).
