Zusammenfassung
Bis zum Kriegsende gehörten Lungenfunktionsprüfungen nur in wenigen Kliniken zu den routinemäßig durchgeführten Untersuchungsmethoden. Nach 1945 ergab sich dann mit der sprunghaften Entwicklung der Herz- und Lungenchirurgie die Notwendigkeit für eine verfeinerte Funktionsdiagnostik, um mit größerer Sicherheit kardiale und respiratorische Störungen abzugrenzen, das Operationsrisiko bei Anwendung komplizierter Anaesthesieverfahren abzuschätzen und die Indikation zu den einzelnen Resektionsverfahren zu stellen, besonders auch im Hinblick auf die Frage nach den postoperativen Atemreserven. Nachdem die ersten klinischen Erfahrungsberichte vorlagen, wurde es deutlich, daß auch außerhalb der Thoraxchirurgie die Operationsgefährdung herabgesetzt werden kann, wenn anatomische und funktionelle Schäden an den Atmungsorganen durch eine Funktionsprüfung präoperativ erfaßt werden. So berichtete Vossschulte (1956), daß in der allgemeinen Chirurgie unter 1736 Verstorbenen der letzten 10 Jahre jeder 8. Todesfall einer postoperativen Lungenkomplikation, Lungenembolien ausgeschlossen, zur Last gelegt werden mußte, daß aber durch die gewöhnliche klinisch-physikalische Untersuchung in mehr als 80% dieser Todesfälle die drohende Gefahr einer Lungenkomplikation nicht erkannt worden war und auch nicht erkannt werden konnte.
