Abstract
Altered mechanics of breathing can limit exercise by: The expiratory flow limitation mechanism, the intrinsic airway resistance and/or the static behaviour of the lung and the chest wall. The adaptation of these parameters to exercise was studied in 11 patients with chronic obstructive lung disease and in 6 normal persons for control.

At rest, 7 patients (B-group) showed a high intrinsic resistance during spontaneous breathing and during a slow vital-capacity (VC) maneuver with a pronounced inverse dependence from the actual lung volume, a low VC, and a normal or a little reduced lung elasticity. 4 patients (E-group) had during spontaneous breathing none or only a little increase of the intrinsic resistance and during a slow VC maneuver a less pronounced dependence of this resistance from the lung volume, a only little reduction of VC, and a marked decrease of the lung elasticity.

All patients (B-group < E-group) had a marked decrease of the critical dynamic pleura-pressures (Pmax) in the isovolume-pressure-flowdiagram and therefore a premature limitation of the maximal expiratory flow (Vmax) by an increase of airway compressibility.

During exercise (200 watts) the normal persons did not reach Pmax and Vmax of the ventilated lung volume. During exercise (50 or 100 watts) 7 B-patients and 2 E-patients exceeded Pmax and 7 B-patients and 3 E-patients reached Vmax.

During exercise the B-group presented an increased mean expiratory airway resistance by means of the high ineffective pressures developed after flow limitation had occured. This was not so marked in the E-group.

During exercise the end-expiratory lung volume increased (B-group < E-group) as partial compensation of both of the lung volume related premature flow limitation and increased intrinsic resistance. This mechanism of compensation is limited by the amount of the VC and/or the static forces of the lung and chest wall.

The adaptation to exercise requires a less respiratory power, when Pmax is higher, VC is larger, the lung and chest wall elasticity is lower and/or the intrinsic obstruction is smaller, than when the inverse is the case.
Zusammenfassung
Für die Leistungsbegrenzung bei Störungen der Atemtechnik sind der Limitierungsmechanismus des exspiratorischen Flows, der endobronchiale Strömungswiderstand und/oder die statische Lungen-/Thoraxwandcharakteristik von Bedeutung. Die Adaptation dieser Parameter an die Bedingungen der Belastungsventilation wurde bei 11 Patienten mit chronisch obstruktiver Lungenerkrankung im Vergleich zu 6 Normalpersonen untersucht.

In Ruhe hatten 7 Patienten (B-Gruppe) bei Spontanatmung und langsamer Atmung der Vitalkapazität (VC) eine hohe endobronchiale Resistance mit ausgeprägter inverser Abhängigkeit vom aktuellen Lungenvolumen, eine stark erniedrigte VC und eine leichte oder fehlende Verminderung der Lungenelastizität. 4 Patienten (E-Gruppe) zeigten bei Spontanatmung keine oder nur eine leicht erhöhte endobronchiale Resistance, bei langsamer Atmung der VC eine weniger ausgeprägte Abhängigkeit der Resistance vom aktuellen Lungenvolumen, eine leicht erniedrigte VC und eine stark verminderte Lungenelastizität.

Im Isovolume-Pressure-Flow-Diagramm hatten alle Patienten (B-Gruppe<E-Gruppe) bei zunehmender Forcierung der Ausatmung eine deutliche Erniedrigung der kritischen dynamischen Pleuradrucke (Pmax) und damit eine vorzeitige Limitierung des maximalen exspiratorischen Flows (Vmax) durch erhöhte Atemwegskompressibilität.

Unter einer 200-Watt-Belastung wurde bei keiner Normalperson im Bereich des ventilierten Lungenvolumens während der Exspiration Pmax oder Vmax erreicht. Unter einer 50- oder 100-Watt-Belastung überschritten alle 7 B- und 2 E-Patienten Pmax. Der exspiratorische Vmax wurde bei allen 7 B- und bei 3- E-Patienten erreicht.

Der exspiratorische Bronchialwiderstand stieg, vorwiegend zu Lasten eines überschießenden Druckaufbau, nach erfolgter Flow Limitierung unter Belastung im Mittel bei der B-Gruppe deutlich stärker an als bei der E-Gruppe.

Im Sinne einer Teilkompensation der jeweils volumenabhängigen vorzeitigen Flow Limitierung und endobronchialen Resistanceerhöhung resultierte unter Belastung bei den Patienten (B-Gruppe< E-Gruppe) eine Verschiebung des endexspiratorischen Volumenniveaus zur Inspiration. Dieser Kompensationsmechanismus wird durch die Größe der VC und/oder die statische Retraktion der Lunge und des Thorax begrenzt.

Die atemmechanische Adaptation an eine Belastung erfordert eine kleinere Atemleistung, wenn Pmax höher, die VC größer, die Lungen/Thoraxelastizität niedriger und/oder die endobronchiale Obstruktion geringer ausfällt als bei umgekehrter Konstellation.
