Abstract
The relationship between heart rate and different ergometric work loads is often used as a simple method to estimate systemic and pulmonary cardiovascular function. However in several patients with additional bronchopulmonary diseases the actual physical work capacity may rather be limited by respiratory than cardiogenic factors. Thus right heart catheterization still provides the most accurate evaluation of pulmonary circulation and right heart function. The technique used for catheterization should always enable the measurement of pulmonary “wedge pressure” to separate active from passive forms of hypertension. Furthermore the most important values of pulmonary circulation in normal individuals are presented as a function of age.
The knowledge of the pulmonary hemodynamics is of actual clinical importance in the preoperative evaluation of patients undergoing thoracic surgery. According to previous investigations in a further series it has been demonstrated that patients with normal preoperative function at rest and during moderate exercise also had normal hemodanimics after pneumonectomy; whereas a preexisting pulmonary hypertension as a rule exceeded markedly after operation. An extensive investigation of the pulmonary circulation should include the differentiation into irreversible and reflex-induced, reversible components of a pulmonary hypertension with regard to some important clinical implications.
In fact an obvious limitation of physical work capacity by an impaired pulmonary circulation is merely present in severe pulmonary hypertension with acute or decompensated right heart failure; in contrast to most patients with stable and chronic cor pulmonale which first are limited by an abnormal work of breathing with marked exertional dyspnoe and respiratory insufficiency.
Zusammenfassung
Von den einfacheren Methoden zur Beurteilung des Lungenkreislaufs hat sich unter anderem die Bestimmung der Herzfrequenz unter verschiedenen Belastungsstufen bewährt. Bei Patienten mit Erkrankungen der Atmungsorgane wird indessen die Belastbarkeit nicht selten durch die respiratorische Insuffizienz vorzeitig eingeschränkt, bevor eine kardiovaskuläre Störung am inadäquaten Anstieg der Pulsfrequenz erkennbar wird. Für eine differenzierte Beurteilung des Lungenkreislaufs eignet sich deshalb besser die Untersuchung der Verhältnisse durch den Rechtsherzkatheter in Ruhe und unter einer leichten Belastungsstufe am Ergometer. Die Meßtechnik sollte unbedingt auch die Bestimmung des pulmonalkapillären Verschluß druckes ermöglichen, damit die klinisch bedeutsamen Formen der aktiven, passiven und hyperdynamen Hypertension differenziert werden können. Bei der Beurteilung muß ferner die Altersabhängigkeit der Druckwerte beim Gesunden mitberücksichtigt werden.
Von besonderer praktischer Bedeutung ist die Kenntnis der pulmonalen Hämodynamik bei der präoperativen Abklärung von Patienten vor einer thoraxchirurgischen Res ektionsbehandlung. Bisher hat sich jedenfalls gezeigt, daß bei präoperativ normaler Hämodynamik in Ruhe und unter leichter Belastung auch nach einer Pneumonektomie keine relevanten Funktionsstörungen auftreten werden; im Gegensatz zu den Patienten mit vorbestehender pulmonaler Hypertonie, wo jede Lungenresektion den Hochdruck erheblich verstärkte. Bei gutachterlichen Fragen empfiehlt sich eine kurzfristige Sauerstoffatmung, um fixierte von funktionellen Widerstandserhöhungen im Lungenkreislauf abgrenzen zu können.
Eine manifeste Beeinträchtigung des täglich geforderten Leistungsvermögens durch eine abnorme Funktion des Lungenkreislaufs ist praktisch nur bei schwerer pulmonaler Hypertonie mit Zeichen der Herzinsuffizienz gegeben. Bei den normalerweise zur Untersuchung gelangenden Patienten wird die physische Aktivität primär meist durch respiratorische Störungen begrenzt, so daß der kardialen Komponente vorwiegend theoretisches Interesse zukommt.
