Zusammenfassung
Das rheumatische Fieber (Polyarthritis acuta rheumatica) stellt eine entzündliche Reaktion des Mesenchyms dar, welche zu einem variablen klinischen Bild führt und durch Rezidivneigung ausgezeichnet ist. Die Erkrankung tritt 1–4 Wochen nach einer Infektion mit β-hämolysierenden Streptokokken der Gruppe A auf. Nur bei einer Chorea minor als Manifestation ist das Intervall zwischen Streptokokkeninfektion und Krankheitsausbruch in der Regel länger. Unter den Streptokokken der Gruppe A sind offenbar nur bestimmte Serotypen in der Lage, den rheumatischen Prozeß in Gang zu setzen [5]. Autoimmunprozesse spielen eine pathogenetisch noch nicht endgültig geklärte Rolle [3]. Bei Ausbruch der Erkrankung sind die β-hämolysierenden Streptokokken im Rachenabstrich häufig noch nachweisbar; der Antistreptolysin-Titer ist im Serum regelmäßig erhöht.
