„Geographic Miss“ in der vaskulären Brachytherapie

Zusammenfassung

Die Anwendung ionisierender Strahlung als therapeutische Option für die Behandlung von Restenosierungen nach Gefäßverletzungen hat in der letzten Zeit zunehmendes Interesse gefunden. Es wurde eine Vielzahl von Methoden zur Verabreichung einer geeigneten Strahlungsdosis an die Gefäßwand entwickelt. Strahler mit niedriger Aktivität basieren auf einer Plattform (z.B. einem Stent), die in das Gefäß implantiert wird und so die langanhaltende (bis zu mehreren Wochen dauernde) Verabreichung einer Strahlendosis ermöglicht. Strahler mit hoher Aktivität werden eingesetzt, um üblicherweise sofort nach der Angioplastie während einer einzigen Sitzung die gewünschte Dosis zu verabreichen. Diese Strahler können als Streifen, Strahlerkette oder Flüssigkeiten vorliegen und Beta- oder Gammastrahlung abgeben. Eine externe Bestrahlung zeigt ebenfalls Wirkung; bedingt durch die ständige Bewegung der Koronargefäße kann diese derzeit jedoch nur bei peripheren Gefäßen eingesetzt werden. Die Wirkungsprofile der einzelnen verwendeten Modalitäten und Isotope zeigen gewisse Unterschiede, und obwohl bei jedem Verfahren die verordnete Dosis an der vorgesehenen Stelle verabreicht werden kann, können diese Unterschiede auch unterschiedliche Komplikations- und Fehlerraten bedingen.
