Zusammenfassung
The typical lung lesion of the Welsh coal miner has been described by Gough (1940), Belt and Ferris (1942) and by Hepplestone (1953). It consists of focal accumulations of coal dust around the respiratory bronchioles. A minimal amount of fibrous tissue encloses the coal and in the immediately adjacent lung there is focal emphysema. These lesions occurring all through the lungs can give rise to fine mottling in the X-ray film and in the patient they produce breathlessness. In miners who have been exposed to a very dusty environment for a long time these lesions are larger and more numerous and produce more emphysema. Generally, however, this type of so-called simple pneumoconiosis is not fatal and does not progress after dust exposure ceases. In some miners, however, a quite different lesion is superimposed. This takes the form of one or more large areas of dense fibrosis enclosing sufficient coal to make them black. These foci increase in size in men who are no longer exposed to dust and are, therefore, spoken of as progressive massive fibrosis (fig. 1 and 2). This is a serious disease leading to right heart failure and death. It is with the etiology of these lesions that we are concerned. Although progressive massive fibrosis only occurs in miners who have already had simple focal pneumoconiosis it is not correlated with the amount of coal present, i. e. it is not due to simple over-dosage.
