Zusammenfassung
Die medikamentöse Konversion atrialer Tachyarrhythmien in den Sinusrhythmus erfährt zahlreiche Limitationen. Der Einsatz von Antiarrhythmika bei dieser Indikation kann mit einer erheblichen Anzahl von unerwünschten und zum Teil gefährlichen Nebenwirkungen einhergehen. Hierbei sind die negativ inotropen Effekte der verschiedenen Substanzen zu berücksichtigen, ihre hemmenden Wirkungen auf die intrakardiale Erregungsbildung und -ausbreitung, die oft erst nach Wiederherstellung des Sinusrhythmus ernsthafte Probleme bereiten können [8], sowie die Möglichkeit von proarrhythmischen Effekten auf Vorhof- und Ventrikelebene [27, 38]. Elektrische Maßnahmen zur Terminierung von anhaltenden atrialen Tachyarrhythmien sind unter Verwendung optimaler Techniken [11, 19] und bei Beachtung anerkannter Indikationen und Kontraindikationen [29, 30, 45 ] sicher, effektiv und mit hoher Patientenakzeptanz einsetzbar. Während bei Patienten mit beständigem reinen Vorhofflattern die schnelle Vorhofstimulation mittels eines transvenös in den rechten Vorhof eingebrachten Elektrodenkatheters das elektrische Verfahren der Wahl zur Wiederherstellung des Sinusrhythmus darstellt [21], haben herkömmliche Stimulationsverfahren keine Effektivität bezüglich der Terminierung von Vorhofflimmern. Bei Patienten mit Vorhofflimmern bietet sich die externe transthorakale [30] oder intrakardiale [2, 24, 25, 32] Gleichstrom-Kardioversion als anerkanntes Verfahren zur Elektrokonversion an. Bei Einsatz dieser nichtpharmakologischen Methoden kann bei den meisten Patienten nach Wiederherstellung des Sinusrhythmus eine deutliche Verbesserung der Hämodynamik erzielt werden [42]. Da im weiteren Verlauf in der Regel mit einer hohen Rezidivrate zu rechnen ist [1, 10, 15, 43], besteht zur Zeit keine einhellige Meinung bezüglich der Indikation zur Elektrokonversion im Einzelfalle. Bei Patienten mit chronischem Vorhofflimmern wird in derzeit noch laufenden Studien [17, 39] das Nutzen-Risiko-Verhältnis zweier unterschiedlicher therapeutischer Vorgehen evaluiert: Wiederherstellung und Aufrechterhaltung des Sinusrhythmus versus Kontrolle der Kammerfrequenz bei fortbestehender atrialer Arrhythmie. Das zukünftige Management von Patienten mit Vorhofflimmern wird sicherlich durch die Ergebnisse dieser Untersuchungen beeinflußt werden. Im vorliegenden Kapitel wird schwerpunktmäßig auf die technische Durchführung der konventionellen transthorakalen hochenergetischen und der niederenergetischen internen Kardioversion von symptomatischem persistierenden Vorhofflimmern eingegangen.
