Zusammenfassung

Neben der antiarrhythmischen Therapie bei Vorhofflimmern kommt der Antikoagulation eine wichtige Bedeutung zu. Denn Morbidität und Mortalität bei Vorhofflimmern werden entscheidend durch die embolischen Komplikationen bestimmt: das Risiko von Thromboembolien ist bei nichtrheumatisch bedingtem Vorhofflimmern 5,6fach, bei Vorhofflimmern auf dem Boden eines rheumatischen Herzklappenfehlers gar 17,6fach erhöht [19]. Als pathophysiologischer Faktor scheint neben der Stase im linken Vorhof, hervorgerufen durch eine herabgesetzte Vorhofkontraktilität, auch eine Hyperkoagulabilität eine Rolle zu spielen [17]. Etwa 75% aller Thromboembolien betreffen zudem das zerebrale Stromgebiet. Daher ist verständlich, daß 6–24% aller ischämischen Insulte auf eine kardiale Embolie bei Vorhofflimmern zurückgeführt werden [8]. Mehrere epidemiologische Analysen und großangelegte Studien haben in den letzten Jahren zum besseren Verständnis der Risikofaktoren für embolische Komplikationen geführt und den Stellenwert der Antikoagulation bei Vorhofflimmern untersucht. Dabei muß zwischen der chronischen Embolieprophylaxe und der Antikoagulation vor und nach Kardioversion unterschieden werden.
