Zusammenfassung
Vorhofflimmern stellt bei Schrittmacherpatienten ein nicht unerhebliches Problem dar. Bei der Sinusknoten-Erkrankung ist es die häufigste Rhythmusstörung überhaupt und wird als ein Endpunkt in der Krankheitsentwicklung angesehen. Bei den AV-Überleitungsstörungen bedeutet das Auftreten von Vorhofflimmern immer eine rhythmologische Zweiterkrankung. Die Häufigkeit des Vorhofflimmerns wird von der Art der Schrittmacherversorgung mit beeinflußt. Die Inzidenz liegt unter asynchroner Ventrikelstimulation höher als unter vorhofbeteiligter Stimulation. Für die vorhofsynchronen Zweikammersysteme bedeutet das Auftreten von Vorhofflimmern immer einen Funktionsverlust und eine Störung der Steuerungsfunktion. Neue Mode-Switch-Konzepte sollen den Schrittmacher in die Lage versetzen, die notwendigen Modusänderungen selbstständig vorzunehmen. Damit wird der Einsatz vorhofsynchroner Ventrikelstimulation auch bei Patienten möglich, die intermittierendes Vorhofflimmern und AV-Blokkierungen aufweisen, bzw. wegen paroxysmaler Tachyarrhythmien einer AV-Knoten-Ablation unterzogen wurden.
