Zusammenfassung

Die wissenschaftliche und versorgungspolitische Bedeutung epidemiologischer Fragestellungen kontrastiert auffällig mit dem Urteil vieler klinisch tätiger Ärzte, aber auch mancher Grundlagenforscher. Für Kliniker erscheint die Beschäftigung mit epidemiologischen Fragestellungen therapeutisch nicht unmittelbar relevant und wird daher nicht selten als bloßes Fallzählen („Erbsenzählen“) abgetan. Dabei wird zum einen übersehen, dass nur auf der Grundlage von Studien zur Prävalenz und Behandlungsbedürftigkeit von Krankheitsbildern versorgungspolitische Entscheidungen getroffen werden sollten, die überhaupt erst die Voraussetzung für adäquate medizinische Versorgung und damit auch für die klinische ärztliche Tätigkeit bilden, und dass die Weiterentwicklung von Versorgungsbedingungen eine genuine ärztliche Aufgabe darstellt. Zum anderen liefern epidemiologische Verfahren in der Versorgungspraxis der klinischen Medizin wichtige Erkenntnisse, z. B. hinsichtlich der Wirkung bestimmter Einflussgrößen auf Behandlungsparameter (wie z. B. Therapieergebnis oder Behandlungsdauer). Aber auch unter (biologisch orientierten) Grundlagenwissenschaftlern ist die Entwertung epidemiologischer Forschungsmethoden nicht selten, da kein auf den ersten Blick erkennbarer Zusammenhang mit biologischen Mechanismen besteht. Hierbei wird übersehen, dass es häufig erst die Ergebnisse analytisch-epidemiologisch angelegter Studien waren, die die Forschung nach biologischen Ursachen von Erkrankungen vorangebracht haben. Eines der besten Beispiele hierfür ist der Zusammenhang zwischen Tabakkonsum und Lungenkrebs. Bei genauerem Hinsehen ist epidemiologische Forschung eng mit klinischen und grundlagenwissenschaftlichen Fragestellungen verbunden und ein unverzichtbares Teilgebiet der medizinischen Wissenschaft.
