Zusammenfassung
Die Größe von koronaren Thromben reicht von fokalen Plättchenaggregaten, die nur mit dem Rasterelektronenmikroskop zu entdecken sind, bis zu einer Masse, die groß genug ist, einen Hauptkoronararterienast zu verschließen. Die ersteren sind wahrscheinlich bei allen Menschen sehr häufig, die fortgeschrittene atheromatöse Plaques haben [1]. Thromben, die groß genug sind, um direkt durch die Veränderung des Blutstroms klinische Symptome zu erzeugen, können angiographisch und/oder angioskopisch nachgewiesen werden. Bei Sektionsuntersuchungen kann man sie anhand von histologischen Serienschnitten rekonstruieren, um die Struktur der dar-unterliegenden Intima darzustellen und die Faktoren, die die Thrombose initiierten, aufzuhellen [1]. Diese Rekonstruktionen zeigen, daß sowohl tiefe als auch oberflächliche Intimaverletzungen eine Thrombenbildung auslösen können. Letztere ist mit einem Endothelverlust („Abtragungsverletzung“) verbunden, bei dem die direkt darunterliegende Bindegewebsmatrix offengelegt wird [1]. Unter 75% der großen intraluminalen Thromben liegen tiefe Intimaverletzungen; diese sind das Ergebnis von Rissen oder Spalten, die sich vom Lumen auf bis tief in die Intima fortsetzen [1]. Plaquefissuren mit darüberliegenden wandständigen Thromben sind die morphologische Basis der angiographischen Typ-II-Läsionen, die bei Patienten mit instabiler Angina [2] und nach Thrombolyse beim akuten Herzinfarkt nachweisbar sind [3].
