Abstract
Based on experience with Beck’s operation [1] it became evident that permanent occlusion of the coronary sinus is detrimental to the myocardium. In 1976, when J. Moll, a cardiac surgeon from Poland, gave a lecture at the Second Surgical Clinic of the University of Vienna on the application of the Beck II procedure, he reported on definitive improvements in some of the patients. Although the audience remained sceptical, certain obvious discrepancies stimulated one of the authors (WM) to initiate further research in this field. This research was based on an impression that the reported beneficial effects (e.g., as pain relief) were primarily due to retrogradely altered microcirculatory pressure and flow dynamics, rather than being caused by the active retroperfusion or arterialization of the coronary veins. Judging from the research developments during the following decade, this hypothesis still seems viable and remains the rationale for clinical application of PICSO for treatment of myocardial jeopardy.
