Abstract
Our experience at the University of Catania, Sicily, has been to some extent similar to that in countries like South Africa, where first world technologies live close to third world social and health organizations: rheumatic disease is still endemic in Sicily; a proper anticoagulation regimen is still difficult to conduct, especially in inland areas; and patient referral from general practitioners, all the way to cardiac surgery, is still slow and often atypical.
